Why the enrollment form matters

The enrollment form is usually the first document a family fills in for you. It becomes your main reference for who the child is, who can pick them up and what to do if something goes wrong. A clear, complete form saves you from chasing missing details later.

Sections most providers include

• Child details: full name, preferred name, date of birth, home address and start date.
• Parents or guardians: names, phone numbers, email, workplace and preferred contact method.
• Custody and pick-up: who has legal custody, anyone not allowed to pick up, and a list of authorized adults.
• Emergency contacts: at least two people besides the parents.
• Health information: doctor, health card number if used in your area, allergies, medical conditions, medications and immunization status.
• Daily routines: naps, eating, toileting and comfort items.
• Consents: outings, photos, sunscreen and emergency treatment, each with its own signature.

Keeping it organized

Ask families to update the form at least once a year and whenever something changes, such as a new allergy or a new authorized pick-up person. Store completed forms securely, because they contain personal and health information.
